Occupational Therapist (OT) – Home Health | PRN | Flexible Schedule
Anova Care is seeking compassionate and skilled Occupational Therapists (OTs) to join our growing home health team. If you are passionate about helping patients regain independence and improve their quality of life, we would love to hear from you.
In this role, you will provide one-on-one occupational therapy services in the comfort of patients’ homes, helping them safely perform daily activities and maintain independence. This is an excellent opportunity for therapists who enjoy flexibility, autonomy, and meaningful patient relationships.
Key Responsibilities
Conduct comprehensive patient evaluations and develop individualized treatment plans
Provide skilled occupational therapy interventions in the home setting
Educate patients and families on therapeutic techniques and safety
Monitor patient progress and document outcomes in the EMR
Communicate effectively with patients, caregivers, and the interdisciplinary care team
Ensure compliance with home health regulations, physician orders, and documentation standards
Participate in care coordination and quality improvement initiatives
Collaborate with clinical and administrative teams to ensure excellent patient care
Perform additional duties as assigned to support patient outcomes
Qualifications
Master’s or Doctorate Degree in Occupational Therapy
Active state licensure as an Occupational Therapist
1–3 years of clinical experience in home health, outpatient, or rehabilitation preferred
Strong patient assessment and treatment planning skills
Knowledge of home health regulations and documentation requirements
Experience with EMR systems and Microsoft Office
Excellent communication and interpersonal skills
Ability to work independently and manage a patient caseload
Schedule
PRN | Pay-Per-Visit
Flexible scheduling with availability during business hours and occasional after-hours/weekend coverage.
Pay
$70 – $130 per visit
